PERSONAL ACCIDENT

-Insurance policy
-If the license plate number is not specified in the policy, a copy of the license of the insured vehicle
-Certified example of traffic accident detection report
-TC number of the payee / victim
-Tax plate (Legal persons) (In case of death, tax numbers of the heirs are requested as tax numbers.)
-Request letter

In the case of Death:

  • Death report and burial license

  • Determination of heirship

  • Example of a registered population record

In the case of Injury:

  • Doctor and hospital reports

  • Invoice and receipt originals related to treatment expense

  • Doctor's prescription for medicine costs invoice received from the pharmacy on behalf of the patient depending on the prescription (including drug price clippings)

For Permanent Disability (Permanent Disability):

  • Delegation Report for Disability

  • If the insured has paid the indemnity as a result of the court decision, the insured is required to submit a court decision, certified copy of the accident report, license photocopy, enforcement order, and payment certificate. 

​

An additional document may be requested by the expert and / or the Insurance Company Damage Department according to the nature of the damage.
